I don't recall any other class action notices I've received ever
requiring such extensive personal information. This seems very fishy...

The word "might" was the qualifier.
As to the notion of the need for personal information, any legal action
of
such nature is going to require such if you are a claimant, so that - in
and of itself - certainly doesn't point to a scam (and besides, there
ways
to check authenticity of a legal suit if that is a concern). As to the
pittance of the reward values, the amounts would have to be multiplied
exponentially to see it as a punitive measure against labels involved in
price-fixing...so the point becomes one of principle (for those who are
inclined) as opposed to a matter of personal monetary gain.
