should make a clarification here... I wouldn't blame Ian/Fugazi for letting it happen. As far as I know you can cover a song without permission as long as you pay the publishing on it....although I could be wrong, as it's not my area of expertise...Sample clearance I can tell you about, cover versions I can not.
